To begin the preschool registration process, parents must first complete the necessary forms on our online registration system- Final Forms.  Completion of these forms as well as uploading all required documents is critical to the registration process. You will be contacted by email at the end of June to let you know if your child has been accepted into the preschool program.

To be eligible to sign up for preschool, your child must be three by August 1st.

• Birth Certificate

• Proof of Residency (dated within the last 30 days)

• Immunization Record

• Physical Form (forms can be downloaded from FinalForms or picked up at the Administration Offices)

• Photo ID of Parent/Guardian

• Income Verification for All Parents/Guardians Living in the Home (i.e. Current pay stub, Unemployment or Social Security benefits, Child Support, or Government assistance)

• Custody Papers (if applicable)

• Current IEP or ETR (if applicable)
